چکیده

Background The NIH estimated that 7 million Americans suffer from coronary artery disease and ~500,000 patients die of heart attacks yearly. Jung et al found that the number of microemboli correlates with the size of myocardium at risk in patients with myocardial infarction [1]. Due to the population inhomogeneity it is difficult to grade and compare LV dysfunction in patients with acute microinfarct and large infarct caused by microemboli and LAD artery obstruction. The objectives of this study were to characterize and compare the effects of LAD occlusion/reperfusion and microvascular embolization on LV function using MRI.

Magnitude and time course of microvascular obstruction and tissue injury after acute myocardial infarction.

BACKGROUND Microvascular obstruction within an area of myocardial infarction indicates worse functional recovery and a higher risk of postinfarction complications. After prolonged coronary occlusion, contrast-enhanced MRI identifies myocardial infarction as a hyperenhanced region containing a hypoenhanced core. Because the time course of microvascular obstruction after infarction/reperfusion is...

Temporal changes of microvascular obstruction and infarct border zone after acute myocardial infarction assessed by contrast enhanced magnetic resonance imaging

Background Besides total scar size, scar characterization parameters are emerging as additional prognostic factors after acute myocardial infarction. Of these, microvascular obstruction (MVO) and infarct border zone are the most promising ones. Nevertheless, a MRI scan performed few days after infarction assesses a rapidly changing myocardium. In particular the infarct border zone that is belie...

Detection of increased coronary microvascular permeability with MRI T1 mapping and gadolinium-labeled albumin

Background Conditions such as obesity and diabetes lead to coronary microvascular disease and, subsequently, an increased risk of adverse cardiac events. Increased microvascular permeability is thought to be an early biomarker of coronary microvascular disease. While prior studies have used MRI with gadolinium-labeled albumin, a macromolecular contrast agent, to detect myocardial infarction, we...

Microvascular integrity and the time course of myocardial sodium accumulation after acute infarction.

Loss of membrane permeability caused by ischemia leads to cellular sodium accumulation and myocardial edema. This phenomenon has important implications to left ventricular structure and function in the first hours after myocardial infarction. We hypothesized that during this period of time, after prolonged coronary occlusion and complete reflow, the rate of myocardial sodium accumulation is gov...

Prevalence and extent of infarct and microvascular obstruction following a range of reperfusion techniques in ST-elevation myocardial infarction

Background Cardiac MRI (CMR) provides unique characterization of myocardial injury post acute ST-Elevation Myocardial Infarction (STEMI). It is the gold standard for non-invasive measurement of Infarct Size (IS) and tissue perfusion during STEMI. Microvascular obstruction describes suboptimal tissue perfusion despite restoration of flow in the infarct-related artery (IRA). IS and MVO are indepe...
